Rates and Payment Policies:

 

The weekly fee will not be reduced due to child absences.  Such as no call no show, vacations and illnesses.

 

There will be a $25.00 fee on any returned checks. 

 

The weekly fee is to be paid on Monday of said week.  If that day is a holiday or day off for the provider/parent then the fee is to be paid on the next scheduled day.  If payment is not made a $10.00 late fee will be incurred.  If payment is not received the next scheduled day services will be terminated until the payment & late fee is received.  If payments are not received within 1 week the contract can be terminated.

    

Rates will be subject to a yearly fee increase.

 

OVERTIME

Overtime will be considered as drop off before the normal drop off time and after the normal pick up time. 

 

If parents make prior arrangements of at least two(2) hours before drop off or pick up, overtime will be approved if the provider is available.

 

HOLIDAYS VACATIONS & ILLNESS ABSENCES

 

The following Holidays will be paid and services will not be provided

-New Years Eve                   -Thanksgiving Day

-New Years Day                   -Day after Thanksgiving Day

-Good Friday                     -Christmas Eve

-Memorial Day                    -Christmas Day

-Fourth of July                    

-Labor Day

 

When the holiday falls on a weekend the appropriate Friday or Monday will be taken.  For example if Christmas falls on a Saturday, Friday will be taken.  If it falls on Sunday, Monday will be taken.

 

The provider may take up to three (3) weeks of vacation, 5 sick days and 3 personal days per calendar year.  The provider will give parents at least a two (2) weeks notice of vacation dates.  If possible notice will be given sooner.  In case of an illness or emergency of the provider or her family the parents will be notified as soon as possible. There is no charge in the event of illness, vacation or emergency of the provider. 

 

In the event of an emergency Keith Etherington, Martha Shelton or Evan Etherington will act as a provider.  The provider will help arrange back up care if possible. If back up care cannot be arranged by the provider the parent is ultimately responsible for finding care for their child(ren).   If the provider arranges back up care for your child in the event of an emergency, weekly fees will not be adjusted. 

 

The weekly fee will not be reduced for child absence such as illness; vacations and no call no show.

In the event of a long-term child absence it will be determined at that time whether or not the position will be held open.

ILLNESS POLICY:

 

The parent agrees to notify the provider of a child’s illness or suspected illness and to make other child care arrangements if the child shows any of the following symptoms:

 

Fever

Flu

Severe cough

Severe cold

Vomiting

Diarrhea

Contagious illness

Conjunctivitis (pink eye)

Unusual rash

Rapid or delayed breathing

Stiff neck

Yellowish skin or eyes

Head lice

Too ill to participate in daily activities

Communicable disease

 

The provider will not accept the child for care if any of the above symptoms are present.

 

If the child shows any of the above symptoms while in care, the provider will remove the child from the group and notify the parents or authorized person to pick up the child.  The child is expected to be picked up within one(1) hour from the time of notification to pick up the child(ren).  If the child is not picked up within one hour a late fee of $5.00 for every 15 minutes will be charged. 

 

The child may return once he/she has been to the Dr. and administered at least one dose of medication.

If the child is too ill to participate in daily activities or is still contagious then the child should stay at home.  The parent should use their best judgment when deciding whether or not the child is well enough to join the providers’ home.  The provider has the ultimate say so in whether or not the child may stay.

 

The child is welcome when he/she has a mild cold such as a runny nose or slight cough provided they are still able to participate in the daily activities.

 

The parent agrees to keep their child (ren)’s immunizations up to date in accordance with the laws of the state.   Written verification by the child (ren)’s doctor will be required yearly.

 

The parent shall give the provider current information regarding any known or suspected special need, handicap or health condition.

 

If any child in the providers care become ill or contracts any disease, all parents will be notified.

MEDICATIONS:

The provider will administer oral medication.  All medications must be brought in original containers.

 

Parents will be required to fill out an administration of medication release and dosage form for each medication.

 

Parents will be advised of amounts and time that the medications were administered while in the provider’s care.

Supplies:

 

Parents agree to supply the following items on a daily basis (if needed):

 

Disposable diapers                                              

Diaper wipes

Pull-ups

Training pants

Swim diapers

Infant formula

Bottles

Nipples

Bottle liners

Baby jar food

Diaper rash ointment

Oral-jel

Change of clothing

Jacket/coats/shoes

Special diet food

 


All diaper wipes, diapers or Pull Ups shall remain at my home for use.  A notice will be sent home when supplies run low. 

 

To follow is what clothing is expecting to be brought on a daily basis.

 

Spring:  jacket, shoes

Summer: sun screen, swimsuit, shoes, and hat (if your child will wear one)

Fall: Jacket, shoes, hat

Winter:  Coat, shoes/boots, hat, gloves, snow suit (if it snows)

 

The provider agrees to supply the following items as needed:

 

Crib                                        Stroller                   Exersaucer                            Blankets                                Bibs

Bouncy seat                       Bed Linens              Baby Swing                          Pack & Play          Dishes

                                               

 

 

 

 

EMERGENCIES

In the event of an emergency, the provider will:

-Call 911, if necessary

-Apply emergency First Aid or CPR

-Contact parent

-If the parent cannot be contacted, the other authorized persons will be

 

The parent will be notified ASAP if any major injury or illness occurs.

Information on First Aid, CPR and Choking are posted in my home.

 

Parents assume all financial responsibility for medical care.

DISCIPLINE:

 

Discipline is handled in a positive manner with redirection, encouragement, consistency, explanation, occasionally time-out and/or removal of privileges.

 

Children will not be called names or shamed in any way.  Spanking will not be done even if you request it according to licensing rules.

 

HOUSE RULES:

 

No hitting, kicking, pushing, biting or otherwise hurting another child or the provider

No spitting or name-calling

No screaming in the house

No throwing toys inside or outside of the house.  (Balls are acceptable)

No food or drink in the family room.  All food and drink must be consumed sitting at the kitchen table.

No playing with the blinds

No jumping or standing on the furniture

No banging on the windows

No going in the street

No going past the set outside boundaries

 

*Failure to comply with the rules can result in termination.
